We have joined forces with The Utah Department of Health which is throughout Utah and is a FREE information line connecting families to community resources and child development information. Joining them has given us an amazing opportunity to use researched based screening tools called ASQ and ASQ:SE which screen children for developmental delays. The screenings begin at birth and go through the age of 5 years old.

Our infants and toddlers are assessed every two months and our twos and preschoolers are assessed every six months. We encourage parents to collaborate with us and complete the online questionnaire so we can serve our families more efficiently. Below are the links to the two questionnaires and should take no more than 5-10 minutes to complete. Results from your screening and staff screening will always be provided to you as a parent.
